Nursing Case Management

Nursing Case Management at AMC is a collaborative process that assesses, plans, and coordinates resources for patients, who are chronically ill or medically complex. Case Managers advocate for patients, communicate with other health professionals and partner with community agencies to provide the most effective, cost efficient care to meet an individual's health needs. It is a service provided free of charge to AMC patients who meet specific criteria for on-going follow-up such as:
- need for coordination of services
- history or possibility of readmission
- history of chronic illness
- medically complex cases
- poor social situation
- terminal illness
- high cost of medical care associated with a long length of stay
The goal is to improve the quality of life and improve a patient's movement through the health care system.
The Case Management Department operates Monday - Friday from 8:00 a.m. until 4:30 p.m.
